Your CPAP Mask Shouldn’t Be That Tight

If your CPAP mask leaks during the night, your first instinct may be to tighten the straps.

Then tighten them a little more.

Unfortunately, tighter does not always mean better.

Your CPAP mask is designed to rest comfortably against your face while creating a gentle seal. When the straps are pulled too tightly, the cushion can become compressed or distorted. That may actually create more leaking—the very problem you were trying to fix.

An overly tight mask can also leave you with:

• Red marks that remain after you wake up
• Tenderness around your nose or cheeks
• Skin irritation or breakdown
• Headaches caused by tight headgear
• Enough discomfort to make you dread wearing your CPAP

It is important to remember that some airflow from the mask is normal. CPAP masks have an exhalation vent that allows the air you breathe out to escape. A true mask leak usually feels different. You may notice air blowing toward your eyes, hear the cushion fluttering or “burping,” or feel the seal repeatedly break when you move.

Before tightening the straps again, try a few simple steps:

  1. Lie down in your normal sleeping position.
    The shape of your face and the position of the mask can change when you lie down.

  2. Turn your CPAP on before making adjustments.
    Adjusting the mask while air is flowing allows you to see how it will actually seal during use.

  3. Make small, even adjustments.
    The mask should feel secure, but it should not feel clamped against your face.

  4. Check the mask cushion.
    Facial oils, moisture, makeup, or a worn cushion can interfere with the seal.

  5. Pay attention to the tubing.
    If the tubing pulls on your mask when you turn over, it can break the seal regardless of how tight the straps are.

If you can only stop the leak by making the mask uncomfortable, the problem may not be the straps. The cushion may be the wrong size, the mask style may not be right for your face or sleeping position, or parts of the mask may simply be worn out.

CPAP should not feel like a nightly wrestling match.

Sometimes a small adjustment—or having someone take the time to look at how your equipment fits—can make a meaningful difference in your comfort and confidence.
